SACRAMENTAL CERTIFICATES AND SPONSOR LETTERS

Sacramental Certificates (Baptism, First Communion, Confirmation, Matrimony) will be received with a written request from the individual (providing they are not a minor). In the case of minors ( up to the age of 18 ), parents or guardians will need to submit a written request . Click below to print out a request form. Complete and return it to the Parish Office during office hours (Monday-Thursday 9 a.m.- 4 p.m.; Friday (9 a.m. – 12 Noon). Please note: No request will be taken by phone.

Those needing S ponsor Letters (for Baptism or Confirmation) will also need to submit a written request with the detailed information. Letters will be given for registered parishioners and will be sent directly to the parish where the Baptism or Confirmation will take place. Click below to print out a request form. Complete and return it to the Parish Office during office hours (Monday-Thursday 9 a.m.- 4 p.m.; Friday (9 a.m. – 12 Noon). Please note: No request will be taken by phone.
